RESUMO

AIMS: Describe the local characteristics, methodology and results of the registry of acromegalic patients in Colombia (RAPACO). METHODS: Multicenter, retrospective study based on the registry of acromegalic patients in Colombia: RAPACO. The data collected included: demographics, diagnosis, approximate time of disease evolution, data on weight, height, body mass index (BMI), neck circumference (NC) abdominal circumference (AC) hip circumference (HC) and waist/hip ratio (WHR); clinical and biochemical data at the time of diagnosis, etiology, immunohistochemistry of the tumor and information related to types of treatment. Descriptive analytics were employed. RESULTS: A total of 201 patients (60% females) with an average age at registration of 49.5 ± 14.6 years and an average time of evolution of the disease of 6.96 ± 4.5 years. Average weight was 75.1 Kg ± 12.98, with an average BMI of 28.11 ± 4.33. The most frequent symptoms mentioned at the time of diagnosis were extremity enlargement and headache. The most frequent comorbidity was arterial hypertension in 50.3% of the cases. 78.6% of cases were caused by macroadenoma. 80.1% received surgical treatment, 77.6% were under medical treatment, of which 95.7% were receiving somatostatin analogues. 26.4% of patients were treated with radiation therapy. Of the patients who received any type of clinical treatment, only 2.5% reported biochemical control at registration. CONCLUSION: It is important to recognize the local epidemiological, clinical, biochemical and treatment characteristics in order to assist in further understanding this pathology to implement local measures to improve both the quality of life as well as the prognosis of the patients diagnosed.

Resumen La hiponatremia es el trastorno hidroelectrolítico más frecuente observado en pacientes hospitalizados y es importante resaltar que se ha asociado a morbilidad y mortalidad de estos. Esta entidad representa un proceso fisiopatológico relacionado con una alteración en la homeostasis del agua, en la cual los pacientes presentan síntomas en su mayoría neurológicos, que se correlacionan con el nivel de sodio y el tiempo de aparición del trastorno. Se realizó una búsqueda en las bases de datos PubMed y Lilacs de monografías, artículos de revisión y artículos originales con el objetivo de revisar aspectos sobre la clínica, diagnóstico y manejo de la entidad. Para el diagnóstico, la batería de estudios se solicita en función del contexto clínico, actuando temprano y permitiendo la corrección del trastorno de acuerdo al escenario respectivo. La hiponatremia es un problema médico frecuente que bajo un abordaje práctico y sencillo permite tomar decisiones clínicas de forma oportuna. MÉD.UIS. 2020;33(2):85-93.


Abstract Hyponatremia is the most frequent hydroelectrolytic disorder observed in hospitalized patients and it is important to note that it has been associated with morbidity and mortality in this patients. This entity represents a pathophysiological process related to an alteration in water homeostasis, in which patients present symptoms mostly neurological, that correlate with the sodium level and the time of onset of the disorder. A search of the PubMed and Lilacs databases of monographies, review articles and originals articles was performed with the objective to review clinical, diagnostic and management aspects of this entity. For diagnosis, the clinical laboratory studies are requested depending on the clinical context, acting early and allowing correction of the disorder according to the respective scenario. Hyponatremia is a frequent medical problem that requires a practical and simple approach favoring clinical decisions in a timely manner. MÉD.UIS. 2020;33(2):85-93.

Resumen Introducción: La glucosa es una variable dinámica en el paciente crítico. La hiperglucemia (mayor a 140 mg/dL) es frecuente en este grupo de pacientes, existiendo distintos enfoques terapéuticos para el control adecuado de la misma. Objetivo: Revisar los aspectos clínicos de la glucemia y la importancia del control glucémico en el paciente crítico adulto. } Metodología de búsqueda: En base de datos Pubmed, utilizando los términos MeSH: "critical illness", "insulin infusion", "insulin protocol", "hyperglycemia". Se incluyeron artículos de revisión y originales, en inglés y español. Conclusiones: El manejo de la hiperglucemia en el paciente crítico es un objetivo primordial en el enfoque integral del paciente de la unidad de cuidados intensivos, dada su asociación con mortalidad, morbilidad y estancia hospitalaria. Abstract Introduction: Glucose is a dynamic variable in the critically ill patient. Hyperglycemia (greater than 140 mg/dL) is frequent in this group of patients, and there are different therapeutic treatments for its adequate control. Objectives: To evaluate the clinical aspects of glycemia and the importance of glycemic control in the critically ill adult patient. Methodology: Search in Pubmed database using the MeSH terms: "critical illness", "insulin infusion", "insulin protocol", "hyperglycemia". Original and review articles are included, in English and Spanish. Conclusions: The management of hyperglycemia in the critically ill patient is a primary objective in the comprehensive approach of the patient in the intensive care unit due to its association with mortality, morbidity and hospital stay.MÉD.UIS.2020;33(2): 49-54.

Resumen El magnesio sérico es el "ion olvidado" en la práctica médica: la mayoría de veces no se tiene en cuenta en los estudios clínicos, sus alteraciones tienden a ser ignoradas y su aproximación terapéutica no está definida de forma adecuada. La sintomatologia producto de la hipomagnesemia es inespecífica y su aproximación diagnostica es compleja. Se presenta el caso de una paciente con hipomagnesemia sintomática severa asociada a daño renal por hipercalcemia durante la gestación.


Abstract Serum magnesium is the «forgotten ion¼ in medical practice. Most of the time it is not taken into account in clinical studies, its alterations tend to be ignored and its therapeutic approach is not well defined. The symptomatology produced by hypomagnesemia is nonspecific and its diagnostic approach is complex. We present the case of a pregnant patient with symptomatic hypomagnesemia secondary to renal damage due to hypercalcemia.

Resumen Introducción: Los tumores de células epitelioides perivasculares (PEComas) son neoplasias poco frecuentes de origen mesenquimal. Estos incluyen el angiomiolipoma epitelioide (EAML), una neoplasia con potencial maligno y cuya presentación simultánea con cáncer de tiroides es bastante rara. Presentación del caso: Hombre de 40 años con masa de crecimiento rápido en relación con polo inferior derecho de tiroides e infiltración de tráquea. La TAC abdominopélvica contrastada muestra múltiples masas renales con masa dominante en polo superior izquierdo. El estudio histopatológico de tiroides evidenció una lesión en lóbulo derecho correspondiente a cáncer papilar de tiroides (PTC) de variante de células altas y una segunda neoplasia en el lóbulo contralateral correspondiente a PTC moderadamente diferenciado de variante clásica. En riñón se encontró una lesión correspondiente a EAML, con células de aspecto epitelioide equivalentes al 40%. Discusión: El presente artículo corresponde al primero en la literatura en describir la presentación simultánea de EAML con PTC bilateral sincrónico (SBiPTC). Se describe una vía molecular común, que corresponde a la vía TSC/mTOR, la prevalencia de SBiPTC y los marcadores inmunohistoquímicos para diagnóstico de EAML.


Abstract Introduction: Perivascular Epithelioid Cell Neoplasms (PEComas) are uncommon tumors of mesenchymal origin. These ones include the epithelioid angiomyolipoma (EAML), a tumor with malignant potential and whose simultaneous presentation with thyroid cancer is quite rare. Presentation of case: Forty years old man with fast-growing mass in relation to lower right thyroid pole and tracheal infiltration. Abdominopelvic contrast-enhanced CT shows multiple renal masses with dominant mass in the left upper pole. Thyroid histopathology showed a lesion in right lobe corresponding to tall cell variant of papillary thyroid cancer (PTC) and a second neoplasm in the contralateral lobe corresponding to classical variant of moderately differentiated PTC. In the kidney was found a tumor corresponding to EAML with epithelioid aspect cells equivalent to 40%. Discussion: This article corresponds to the first in the literature to describe the simultaneous presentation of EAML with synchronous bilateral PTC (SBiPTC) and its possible association. A common molecular pathway corresponding to the TSC/mTOR pathway is described, as well as SBiPTC prevalence and immunohistochemical markers for EAML diagnosis.
